MOBILE PHLEBOTOMY SERVICE FEES
QuickPunctureLabs LLC provides mobile phlebotomy and specimen collection services on a direct payment basis.
Our mobile service fee covers:
• Travel to your location
• Appointment scheduling and coordination
• Professional blood specimen collection
• Proper specimen handling and preparation for transport to the laboratory
The mobile service fee is separate from laboratory testing charges and is not billed to or reimbursed by insurance.
LABORATORY TESTING & INSURANCE
QuickPunctureLabs LLC does not perform laboratory testing. Once your specimen has been collected, it is delivered or shipped to the designated reference laboratory for analysis.
The processing laboratory bills your insurance provider directly for laboratory testing. Any deductibles, copayments, coinsurance, or patient balances related to laboratory testing are determined by your insurance provider and the laboratory performing the testing.
Service Fees
Because every appointment is different, service fees may vary depending on the location, travel requirements, and services requested.
To receive a quote for your appointment, please contact QuickPunctureLabs LLC before scheduling. We will gladly explain any applicable service or travel fees before your appointment is confirmed.
Travel Fees
Our standard service area is included in our regular mobile service fee.
Appointments outside our standard service area may require an additional travel fee based on round-trip mileage. Any applicable travel fee will be discussed and approved during scheduling before your appointment is confirmed.
Appointment Availability
We strive to offer flexible scheduling, including:
• Same-day appointments (when available)
• Next-day appointments
• Scheduled visits to homes, workplaces, healthcare facilities, assisted living communities, and other approved locations
• Appointment availability is subject to scheduling and service area coverage.
Payment Responsibility
By scheduling an appointment with QuickPunctureLabs LLC, you acknowledge and agree that:
• Payment for the mobile phlebotomy service is the responsibility of the patient or the person scheduling the appointment.
• Payment is due at the time of service unless other arrangements have been made in advance.
• The mobile service fee is separate from laboratory testing charges.
• Laboratory testing fees, insurance claims, and any remaining patient balances are handled directly by the processing laboratory and your insurance provider.
